Randomized controlled trial of trehalose An efficient autophagic bioprotectant in the management of dry eye disease Morya AK, Solanki K, Prakash S, Samota M, Gupta A, - Taiwan J Ophthalmol
PURPOSE: To compare the therapeutic effect of sodium hyaluronate (SH)–trehalose (Trehalube, Microlabs, Bangalore, India, SH 0.1% and trehalose 3%) or SH (0.1% Hylotears, Raymed, Chandigarh, India) alone in patients with dry eye disease (DED).MATERIALS AND METHODS: Patients were randomized into two groups: SH-trehalose (SH 0.1% and trehalose 3%) or SH (0.1% Hylotears) alone. The Ocular Surface Disease Index (OSDI) questionnaire was used to assess patient's symptoms. Patients were followed up at 4 and 8 weeks, and OSDI score, tear film break-up time (TBUT), tear film height (TFH), Schirmer's test, and conjunctival staining were evaluated at each visit.RESULTS: A total of 384 patients were included in the study, 192 patients in each arm. The mean age of participants was 37.62 + 14.4 years and 225 were women (56%). The improvement in Schirmer's test was significantly better in the SH-trehalose group at 8 weeks (5.26 + 4.3 mm, 95% confidence interval = 4.6–5.9 mm) compared to the SH group (3.71 + 3.9, 95% confidence interval = 3.15–4.28 mm). The TBUT and TFH showed slight improvement at 4 weeks in both groups, but not at 8 weeks. There were no group differences at all-time points in terms of conjunctival staining and OSDI-based grades of DED.CONCLUSION: It was found that treating dry eye with SH-trehalose leads to greater improvement in the Schirmer's values and TBUT after 8 weeks of sustained use in patients with DED, and this was more pronounced in those with severe DED.

Again, I'm trying to give you practical information that you can apply in your day to day life. And so if we ask, I said earlier that one of the things that this drug hydroxycholroquine does is it disrupts viral hijacking. How does it disrupt viral hijacking? So one of the ways that it's suspected that this drug blocks viral hijacking, is that it is known as a zinc ionophore. That means that this drug binds to zinc. So we'll just use hydroxychloroquine as our example, it binds to the zinc and it pushes it into the cell. And then once it pushes it into the cell, it can push it into the cellular machinery, the golgi apparatus, the, the other places in the cell like mitochondria, et cetera, can push that zinc into those compartments in the cell where DNA and RNA replication can occur, which is where generally, the virus likes to get to that component so that it can take over. And so as an ionophore pulls zinc into the cell. So generally speaking, zinc, there are receptors on the surface of the cell and receptors on the surface of the organelles in the cell that are zinc dependent. So the zinc passes through these receptors and, and it's like a, a door that opens and closes. And so, what hydroxychloroquine does is it binds and it pulls it through into the cell and concentrates it into those areas of the cell. So knowing that these medicines can do that, I want to talk a little bit about zinc tonight. Id like to show you that zinc has a role as an antiviral and all the different things that it has the capacity to do. In this video you can see here in abundance of evidence has accumulated over the past 50 years to demonstrate the antiviral activity of zinc against the variety of viruses and via numerous mechanisms. Now this was just recently published. This was a review of the literature. This was just published in 2019 - just a few months ago. I want to show you where zinc plays a role in inhibiting viral replication. Click here to subscribe - https://www.glutenfreesociety.org/wxrn Quercetin is a bioflavonoid. It's a plant based product or chemical that has antiinflammatory properties on that enzyme. Quercetin can have a very nice antiinflammatory effect if you're taking it prophylactically. There's actually research studies that show quercetin has antiviral properties,and that quercetin actually can reduce the risk of developing upper airway diseases, upper respiratory tract infections and things of that nature. Quercetin also blocks the cycle oxygen ACE enzyme. That's why a lot of scientists are focusing in on quercitin as a potential. I wouldn't recommend quercitin to block the COX enzyme to reduce a fever because it doesn't work quite that way. Quercitin is not going to have that acute fever reducing capacity, but it is going to have that that overall reduction in overall inflammatory capacity. Then why do we want to know about this as it relates to any virus? Well, the research shows that quercetin through its effect on inflammation, can inhibit viral replication in multiple different areas. It's been very, very powerful as a nutrient in regards to immune function. So a lot of you have asked me, because vitamin C is very scarce right now, what can I use if I want to support my immune system right now? What can I use instead of vitamin C? I just can't find any vitamin C. Well, one of the things you can use is quercetin. Quercetin is very, very good at a lot of the things that vitamin C is good at. And because of its antiviral properties, it's actually been studied to be antiviral against the rhinovirus, against influenza, against the hepatitis virus, against Ebola virus. So there are a number of research studies that show that course it's in has very potent antiviral effects. Disaccharide glasses are increasingly used to immobilize proteins at room temperature for structural/functional studies and long-term preservation. To unravel the molecular basis of protein immobilization, we studied the effect of sugar/protein concentration ratios in trehalose or sucrose matrixes, in which the bacterial photosynthetic reaction center (RC) was embedded as a model protein. The structural, dynamical, and H-bonding characteristics of the sugar–protein systems were probed by high-field W-band EPR of a matrix-dissolved nitroxide radical. We discovered that RC immobilization and thermal stabilization, being independent of the protein concentration in trehalose, occur in sucrose only at sufficiently low sugar/protein ratios. EPR reveals that only under such conditions does sucrose form a microscopically homogeneous matrix that immobilizes, via H-bonds, the nitroxide probe. We conclude that the protein immobilization capability depends critically on the propensity of the glass-forming sugar to create intermolecular H-bond networks, thus establishing long-range, homogeneous connectivity within the matrix.
